ADMISSION GUIDELINES - CRITERIA FOR ACCEPTANCE
A. A signed copy of Our Lady of Mercy Covenant must be submitted with all applications.

B. Students will be accepted into Our Lady of Mercy School according to the following priorities:
1. Children attending Our Lady of Mercy School promoted to a higher grade
2. Children who are Catholic members of Our Lady of Mercy Parish and registered at Our Lady of Mercy Church.
3. Children who have brothers or sisters attending Our Lady of Mercy School.
4. Out-of-Parish Catholic children
5. Non-Catholic children (These students will participate in the school’s religious program other than the actual reception of the sacraments.)
C. Our Lady of Mercy School admits all qualified students regardless of race, sex, or national origin.
Note: In determining acceptance, the school will review the academic, discipline, and attendance records of students transferring from another school. All new students applying in grades Pre-Kindergarten through eighth are screened as part of the application process.
Acceptance and placement will be determined following the screening and interview process. During the process, a school faculty or staff member will interview the applicant before an admission decision is finalized. Students performing at or above grade level will be considered for enrollment. Additional testing and/or recommendations may be required to evaluate an application and offer acceptance.
DOCUMENTS REQUIRED FOR APPLICATION
In order to expedite the application process, copies of all documents required should be prepared in advance for school records. Applications without the required documents will be considered incomplete and will not be processed until all documents are submitted.
Pre-Kindergarten:
A child must be four years of age on or before September 30th of the incoming school year. Copies of following documents are required:
1. Certificate of Baptism
2. Official State Birth Certificate
3. Immunization Record: This card should be made available to you from your doctor or from the EBR Parish Health Unit. A single dose of HIB vaccine and a single dose of MMR vaccine is required from all children entering Pre-Kindergarten with second dose of MMR at or after age 4.
4. Student's Social Security Card
Kindergarten:
A child must be five years of age on or before September 30th of the incoming school year. Copies of following documents are required:
1. Certificate of Baptism
2. Official State Birth Certificate
3. Immunization Record: This card should be made available to you from your doctor or from the EBR Parish Health Unit.
Immunization Required:
a. Minimum of 4 DPT immunizations
b. 3 doses polio vaccine - last dose after 4 years of age
c. 2 doses of MMR vaccine (measles, mumps & rubella) with last dose at or after 4 years of age
4. Student's Social Security Card
New Students - Grades 1-8th:
For First Grade, a child must be six years of age on or before September 30 of the incoming school year. Copies of the following documents are required:
1. Certificate of Baptism
2. Official state Birth Certificate
3. Immunization Card
4. Student's Social Security Card
5. Most recent report card and Achievement Test scores
6. Any educational or psychological evaluation administered to your child
